Program Policy (English)
* Approval/eligibility vary by fund. Availability of assistance programs are subject to change as they are distributed until funds are depleted. *
- The child’s original diagnosis must have occurred before age 18, and the child must be under age 21 at the time of request.
- SFH cannot reimburse travel expenses, cover travel that has already taken place, or pay for reservations made directly by a parent or guardian.
- Requests for Fuel and Meal Assistance may be submitted up to a maximum of three (3) times per month.
- Requests for Lodging Assistance may be submitted up to a maximum of two (2) times per year.
- Requests for Transportation Assistance, including use of the SFH van, must be submitted at least 2–3 weeks in advance and are subject to availability and approval.
- A valid government-issued ID is required at the time of pick-up for all approved transportation assistance involving the SFH van.
- Additional information or documentation may be requested and will be communicated via email.
- All assistance requests must be submitted at least four (4) days in advance.
Same-day requests will not be approved. - SFH processes request forms within 1–2 business days.
- Fuel and Meal Assistance must be picked up prior to the travel date.
- Pick-up hours are Monday–Friday, 9:00 AM–1:00 PM and 2:00 PM–5:00 PM.

- El diagnóstico original del niño debe haber ocurrido antes de los 18 años, y el niño debe tener menos de 21 años al momento de la solicitud.
- SFH no puede reembolsar gastos de viaje, cubrir viajes ya realizados ni pagar reservaciones hechas directamente por el padre, madre o tutor.
- Combustible y comidas: hasta tres (3) veces por mes.
- Hospedaje: hasta dos (2) veces por año.
- Transporte: solicitar con 2–3 semanas de anticipación; sujeto a disponibilidad y aprobación.
- Se requiere identificación oficial vigente al recoger asistencia de transporte con la camioneta de SFH.
- Podría solicitarse información/documentación adicional (se notificará por email).
- Presentar con al menos cuatro (4) días de anticipación. No se aprueban solicitudes del mismo día.
- Procesamiento: 1–2 días hábiles.
- Combustible y comidas deben recogerse antes de la fecha del viaje.
- Horario de recolección: lunes a viernes, 9:00 AM–1:00 PM y 2:00 PM–5:00 PM.
